Advanced Form Fees

Medical forms vary in scope and scale, we offer two types of Advanced Forms. We can quickly assess your requirements and give a clear quote via this FORM QUOTATION form

Service Set Up (inc GST) Annual Support Fee
Advanced Form (Standard) $220 $55
Advanced Form (Custom) $330 $55
Extra Edits (>15 Min) - $55 per 30 min

Any Form Quotation and the build fee are subject to the clients form request that includes: list of fields, the types of fields, field functions and input flow. Most Advanced Forms cost either $220 or $330 to set-up.



After the initial form design submission, the build fee includes two reviews and change requests. Variations to an initial brief or scoping document for a form or additional reviews that require changes to the form design may incur a fee of $55 per 30 minutes.

Form Support & Maintenance

Advanced Forms are not expressly covered by the standard Website Support and Maintenance Agreement.

Unless otherwise stated.Advanced Form Support may attract extra fees based on changes that may be required.


Change requests that are not resolved in under 15 minutes are charged at $55 per half hour (including GST).
